Yellow-patterned walls, damp carpet and humming fluorescent lights set the tone in The Backrooms, an add-on that adds an infinite, procedurally generated horror dimension. It relies on scripting APIs, so Experimental Features must be enabled in the world settings.

The dimension begins in Level 0, a repetitive, confusing maze with unstable transitional zones, and continues into Level 1, a huge open parking structure with endless concrete and low visibility. Custom entities roam it: the aggressive Smiler thrives in darkness and drops loot, while the slower, unpredictable Duller wanders between zones and gives crafting materials. A Presence Scanner detects nearby entities, and the pack also adds items like Almond Water and furniture such as chairs.
