Sanity Logic Addon (1.21) adds a wakefulness mechanism that doesn’t use a bar or numbers on the screen. Wakefulness levels change based on the environment and actions. Being in the dark, near monsters, or eating raw food reduces wakefulness, causing hallucinations and false noises.
